I think unrated pipe is the only possibilty.

If the pressure was allowed to escape the barrel,  then I would think the explosion to be minimal rather than disastrous. What are the ramifications of using wax in a combustion? I know that the short duration of the burst probably wouldn’t melt it, but I know materials react differently when both heat and pressure are applied to them, such as superplasticity.

i've used NSF-DWV before for a combustion and had my ammo jam when i fired, but all it did was hold the pressure inside. Combustion doesnt generate enough pressure to explode PVC (30-50 PSI) from what I have found.
maybe it was cellular core?
or maybe there was already a crack in the chamber?

The PVC could have had a hairline crack in it. 
I've had a piece of pressure rated PVC fail in a situation where the only explanation I could come up with is that the PVC was weakened by a crack. Luckily it wasn't the pressure vessel or else I would have had a bad day.
However, if I had to guess I would say this was most likely made of drainage pipe.
